Your name has been randomly selected from Georgia Voter Registration or Georgia Department of Driver Services data and will remain confidential. Completion of this questionnaire is required by federal law.

I am locked out of eJuror

Please allow a duration of 30 minutes before attempting to log in again. After this period, you should be able to access the system and complete the questionnaire.

Juror Scam

This mailing is legitimate. Your name has been randomly selected from the Georgia Voter Registration or Georgia Department of Driver Services records. You are being considered for jury service in the Middle District of Georgia. Your responses to the questionnaire will assist us in determining your qualifications for potential juror service over the next four years.

If you have not yet done so, kindly complete the online questionnaire at your earliest convenience.

Can I wait for a paper questionnaire

If you prefer to wait for the paper questionnaire to be mailed to you, that is perfectly acceptable. However, we highly recommend submitting your questionnaire online to ensure timely receipt. If you would like to complete the questionnaire online, please follow the instructions provided above.

The paper questionnaire is damaged or lost

We highly recommend submitting the online questionnaire to ensure timely receipt. If you would like to complete the questionnaire online, please follow the instructions below. If you are unable to complete the online questionnaire another paper questionnaire will be mailed to you.

Questionnaire received for child in college

If the person is away at college, please forward the questionnaire information to them if possible. The quickest and easiest way to complete the questionnaire is online, using the link and information below; however, you may send them the paper questionnaire to complete. If it is not possible to forward the questionnaire or link below, you may complete the questionnaire on their behalf. You should note in the remarks section of the questionnaire that you have completed the form and include the reason for doing so. Also, please specify your relationship to the person. Do not sign the form.

Jr. and Sr. living in same household

If both are registered to vote in Georgia, or if both have a driver’s license issued by the State of Georgia, please provide us with the participant number, and we will send you information regarding who needs to complete the questionnaire.

Questionnaire received for someone in military

If the person is away due to the military, please forward the questionnaire information to them if possible. The quickest and easiest way to complete the questionnaire is online, using the link and information below; however, you may send them the paper questionnaire to complete. If it is not possible to forward the questionnaire or link below, you may complete the questionnaire on their behalf. You should note in the remarks section of the questionnaire that you have completed the form and include the reason for doing so. Also, please specify your relationship to the person. Do not sign the form.

Questionnaire received for someone deceased

Return the questionnaire on that person’s behalf. In the remarks section on the back of the form, please indicate: 1. Write “DECEASED” in the REMARKS section along with the date of his/her death; and 2. Your name and relationship to the individual 3. Death Certificate is NOT needed.

Questionnaire received for someone disabled

Please complete the questionnaire on that person's behalf. A doctor's note is NOT required. In the remarks section on the back of the form, please indicate: 1. The nature of the disability; or 2. A brief explanation of the medical condition in the REMARKS section. 3. Your name and your relationship to the individual.

Doctor's statement

A doctor’s note is not required. We kindly ask you include a brief explanation of the medical condition in the remarks section at the end of the form.

Family member or friend doesn't live here anymore

If possible, forward the questionnaire to the new address. If it is not possible to forward the questionnaire, please write on the questionnaire that the person no longer lives at that address and then return the questionnaire to the Court.

Questionnaire sent to someone who is out of state

If the individual is away from home for a short period (less than two months), you have the option to either forward the questionnaire information to them or retain the questionnaire until their return. Upon their return, they can complete the questionnaire at that time.

If the individual will be away for more than two months and you are unable to forward the questionnaire, kindly note this information on the questionnaire and return it to the Court.

I am 70 or older

In our court, if you are 70 years of age or older and would like to be excused from serving as a juror, we still ask that you return the questionnaire to us. However, you only need to provide your age and date of birth in Question No. 2 on the questionnaire; also, at Question No. 14 – Grounds for Requesting Excuse, you should color in the oval shape beside “1.” That will let us know that you are requesting to be disqualified based on your age. Please then sign the form and return it to the Court. You may also complete the questionnaire online using the information below.

Different address or non-Georgia resident now

We kindly ask that you complete the questionnaire and update your address. Please be sure to provide your information in the designated name and address section.
